Do I need to make an appointment?

It is recommended that you make an appointment for medical services, but you may call the clinic to see if there are any available walk-in appointments for that day. Sometimes people do not show up for their appointments, so you are welcome to come in and wait and we will do our very best to get you in for an appointment. If it is an emergency, please call 911, go to the local Urgent Care Center or the Emergency Room. The SMDC Nurse-On-Line # is 786-3110 or 1-800-714-4646.

You must call to schedule an appointment for dental services. 

What if I need to cancel or change the appointment?
Please call us at least 24 hours in advance or as soon as you know you cannot make your appointment. Another patient may be able to use your appointment time. Also, please call us if you're running late, since we may need to reschedule your appointment.

What do I bring to my appointment?

  • If you have health or dental insurance, please bring the insurance card.
  • ONE item for proof of income such as: a recent check stub, the most recent W2 forms from your taxes, Form 1040 or a letter from Social Security. Proof of income is required at the beginning of every year.
  • A current list of your medications or the bottles of your medications.
  • If your children are being seen at the health clinic, please bring immunization records.
