What if I need information from my medical records?
LSCHC protects the confidentiality of your medical records. That’s why we require you to fill out and sign a form called the Authorization for Use and Disclosure of Protected Health Information. Once we receive a signed copy of this completed form, we usually can process your request within 10 business days.

If your test results are normal, you will receive a letter in the mail with the results. A provider or nurse will contact you if your test results need follow up treatment. If you are uncertain about your test results you can always call our office. At this time you must provide your client id number or your name, birthday, and social security number. Let the receptionist know what test results you are looking for and on what date you had taken the test. If a nurse is available, she/he can give you your results. Otherwise, we will have the nurse give you a call back with your results as soon as possible.
What if I need a referral?
You must see one of our providers and if they determine that you need a referral, you will be given one. There are some programs that pay for specialty care. Our social workers can help you apply for financial assistance to cover the costs of specialty care that you may need.
Are you a free clinic?
We are not a free clinic, however, we do provide health care on a sliding
fee scale based on family size and household income. There are also some programs that pay for certain
services. We want you to get the health care you need, so if you have
concerns about payment, please contact us.
Are all of the doctors volunteers?
No, we employ highly qualified professional physicians and
family nurse practitioners who are able to provide continuity
of care to our clients. We also have some volunteer physicians
who offer specific services and make it possible for us to
