Lake Superior Community Health Center (LSCHC) is a 501(c) 3 non-profit organization. Since its inception in 1972, LSCHC has grown and expanded, and today operates as a federally-qualified health center (FQHC), providing comprehensive primary and preventive health care services for the communities in the Twin Ports. We have clinic locations in Duluth, MN and Superior, WI. We treat the whole family in an integrated care model, regardless of insurance or income.

 

Timeline of Our History

  • 1972 - Duluth Community Health Center (DCHC) was founded in Duluth's Central Hillside

  • 1980 - First expansion of the clinic in Duluth

    Duluth Family Health Clinic 1991

  • 1993 - DCHC became a Federally Qualified Health Center

    Duluth Family Health Clinic 2000

  • 2000 - Now known as Lake Superior Community Health Center (LSCHC), we expanded our services to Superior

  • 2005 - LSCHC relocated to current site in Duluth, and Dental services are added in Superior

  • 2007 - Dental services begin in Duluth

  • 2013 - LSCHC became a Certified MDH Health Care Home

  • 2020 - New Dental Clinic is opened in Carlton

  • 2020 - New Chiropractic Clinic opens in Duluth

  • 2022 - New Clinic in Superior opens on 2222 E. 5th St

  • 2022 - After 50 years of serving the community, we are celebrating our 50th Anniversary this Fall. Stay tuned for updates!

This health center receives HHS funding and has Federal Public Health Service (PHS) deemed status with respect to certain health or health-related claims, including medical malpractice claims, for itself and its covered individuals.
