Insurance Navigator Videos
Our Health Care Access Office Staff can help you sign up for health insurance. July 20th mammogram Event

On July 20th we are partnering with SAGE and American Cancer Society to offer mammograms to female patients 50 and older at our Duluth location. Women who complete their mammogram will recieve a giftbag and be entered into a drawing for a giftcard! Uninsured Minnesota residents of appropriate screening age will be elligible for a free mammogram covered by SAGE. We have a few spots left to fill, so if you are due for your mammogram please call sage to sign up: 1-888-643-2584!

COLORECTAL CANCER SCREENING

If you are 50 years old or older, you should be screened for colorectal cancer.

Are you looking for an alternative method of colorectal cancer screening that does not involve any prep?? We offer Fecal Immunochemical Tests (FIT). CLICK HERE to see how easy it is. Contact your provider to find out how to get one.

 

nominal fee update for sliding fee patients
There will be an increase to the nominal fee effective March 1, 2015. Medical nominal fees will increase from $5 to $10. Dental nominal fees will increase from $20 to $25. 




State of the Art Behavioral Health Care is now available at LAKE SUPERIOR COMMUNITY HEALTH CENTER


Integrated Behavioral Health Care (IBH) coordinates your physical and behavioral health care at Lake Superior Community Health Center. Physical health and behavioral health concerns can happen at the same time. Often, your mental health affects your physical health and your physical health can affect your mental health. Providing you with services to treat both will have the best results for your overall health and well being.

Lake Superior Community Health Center offers IBH along with your primary care. Your medical provider will talk with you about any behavioral health needs you have and may offer you IBH services or you may request to meet with the behavioral health clinician at your medical visit.

IBH is provided by our trained behavioral health specialists. Often, they are able to see you on the same day that you see your medical provider. Visits are 15-20 minutes long. You will have a chance to talk about life changes and stressors. Helpful information will be provided to you by:
 

 

  • Behavioral Health Clinicians, for your mental health needs: anxiety, depression, bipolar disorder, stress management, life changes, adjustment to medical conditions, grief, loss, problem solving, coping skills, information and referral to community resources.


  • Health Coach, for help with diet, exercise, lifestyle changes, diabetic management, and smoking cessation.


  • Social Workers, from the Health Care Access Office can help you apply for health insurance and tell you about where you can get help with health care advocacy, housing, food, meals, and clothing.

A follow up visit may be recommended or referrals might be made depending on your individual needs.

Patient Experience Survey 
It is that time of year again when we ask for patient feedback! If you are dental patient, we will be handing you a survey in the office. If you are a medical patient, you may be recieving a survey in the mail.

We highly encourage you to complete the survey so that we know what we are doing well and where we can improve. We are constantly working on improvement projects and want to ensure we are prioritizing the needs of our patients.

Open enrollment 

As of January 1, 2014, it became federal law under the Affordable Care Act that everyone has health insurance. If you do not have health insurance, you may face a financial penalty.

Our Health Care Access Office staff is here to help you navigate the health insurance plans that are available to you and help you enroll...for FREE!
