ShareCare
ShareCare is a local program that allows individuals to pay an affordable rate for health services based on their income. The program is available at several Duluth/Superior area clinics and at St. Luke’s Hospital. A ShareCare member card is required prior to receiving services at the participating organizations. MinnesotaCare
MinnesotaCare is a subsidized health care program for people
who live in Minnesota and do not have access to affordable

Medicaid / Medical
Assistance
Minnesota’s Medicaid program provides necessary medical
services for low-income families, children, pregnant women,
and people who are elderly (65 or older) or those with disabilities.
Minnesota Medicaid is also known as Medical Assistance. Click here for eligibility and benefits information.
GAMC
Minnesota’s General Assistance Medical Care (GAMC) Program
pays for medical care for Minnesotans who don’t
qualify for Medicaid or other state or federal health care
programs - primarily low-income adults between the ages of
21- 64 who don’t have dependent children.

SMDC Community Care
The SMDC Community Care Program is available to all individuals who
are uninsured and unable to pay for their medical services. Eligibility
is determined by measuring family income and assets against current
guidelines. Some services may not be eligible under this program. Contact

Medication Assistance
For Non-Health Center Clients, our staff will help people
fill out the paperwork for medication program assistance. We're able to access much of the information online and get clients started
in the direction of assistance and prescriptions. Health Center clients
get help with the medication program assistance paperwork from our Medical
Staff.
Lion’s Club Referrals
Our staff will make appropriate referrals to
the area Lion’s Club for help with eye glasses and hearing exams.
Minnesota SAGE Program
The SAGE Program (formerly the Minnesota Breast and Cervical Cancer
Control Program - MBCCCP) provides free breast and cervical cancer screening
and follow-up services to uninsured and underinsured women over the
age of 40. You can sign up for this program at the Health Center; call
722-1497 to make an appointment.

Minnesota Family Planning Program
Women or men between the ages of 15 – 50 can receive free family planning office visits and education, birth control, sterilizations, and diagnosis and treatment of sexually transmitted diseases found during a family planning visit. Click here for eligibility and benefits information.
